How to Request Blood Components?
To obtain blood or blood components from our facility, hospitals must provide a signed requisition form. Please follow these steps to secure blood components:
- Get the official Blood Requisition Form filled and signed by the treating medical officer at your hospital/clinic/nursing home.
- Ensure a blood sample tube from the patient (labeled with name and hospital details) is sent along with the form for cross-matching, along with labelled patient sample provided by the hospital.
- Bring the form and patient sample to the Rotary Blood Centre Jodhpur (open 24/7 for emergency supplies).
